Explanation of the Property Survey Form

What does the "Requested Dispositions" (item #4) on the "Request to Survey Equipment" form mean?

  • Surplus Sale -Items marked for Surplus Sale will be picked up by Facility Services after the Survey Form has been approved, and will be sold.
  • Salvage/Parts - Equipment marked "Salvage/Parts" means the department that owns the equipment is salvaging the equipment for its internal parts, and it will no longer maintain its original form. When approved, the original equipment will be deleted from the department's inventory
  • Junk - Equipment marked "Junk" was considered valueless and the owning department has thrown it away, which violated Cal Poly equipment disposal procedure guidelines. The prescribed procedure was for the department to survey the equipment as "Surplus Sale," or "Salvage/Parts".
  • Missing/Lost - The equipment cannot be found within the department.  In addition to the "Request to Survey Equipment" form, the department must fill out an "Equipment Loss Report," and send both to the Property Office. If equipment has been missing for two Inventory Cycles, the Property Office supervisor will contact the University Police.
  • Stolen - "Stolen" should only be checked if there has been a known break-in.  In this case, the Property Office supervisor will notify the University Police, who will fill out a Police report. The Police will issue a case number for this stolen equipment.
  • Destroyed (fire, etc.) - Equipment which has been rendered useless because of fire, water, or other damage should be marked "Destroyed". It will be picked up by Facility Services when the survey request has been approved.
  • Trade-in (provide source and price) - Choose "trade-in" for equipment that is sent in to a vendor for replacement or upgrade.  Include the name of the vendor who will be accepting the equipment, and what you will be getting in return (a newer monitor, money, etc.).  Note that this is not to be used for leased equipment.
