Authorization
to Release Student Account Information
List all persons to whom a student's account
information can be released. Student must sign.
Authorization
for Transmittal of Funds
For students in off-campus programs who want
their financial aid funds forwarded.
Financial
Aid Direct Deposit Authorization
The direct deposit sign-up process is faster when
you use your portal access. Go to the Money Matters tab and scroll
to the DIRECT DEPOSIT NEW/EDIT link.
PLUS
Borrower Authorization
To request that excess parent PLUS loan funds be
direct deposited to the student's direct deposit account. Parent
must sign.
Prior
Term Payment Authorization
To have a prior term balance paid by your financial
aid.
Subject to approval by the Student Accounts Office.

Request Form
Complete the STUDENT INFORMATION box only.
