Employee Fee
Waiver: How to Apply
If you will be enrolling
in work-related or career development courses only, you are not
required to matriculate into a degree program. Complete a paper
CSU undergraduate or post-baccalaureate/graduate application and
submit it to the Human Resources Office. Complete all fields of
the application or call the Human Resources Office at ext. 6-7246
or the Admissions Office at ext. 6-2313 for specific guidance. The
“name of your intended major” should be ‘undeclared’.
You do not need to enter a major code.
To obtain a paper application, you may either go to the Admissions
Office (Admin. Bldg., Room 206 – hours are Monday-Friday,
8 am to 5 pm, except campus holidays) or you may choose to download
the paper
application
located on the CSUMentor website. Once at the website, go to the
“Ask an Expert” tab, select “Student Support”,
select the “CSUMentor Technical Support”.
If you plan to matriculate
into a baccalaureate degree program (i.e. BA, BS), you must use
the online application.
Go to the “Apply Online’ tab and follow the instructions.
You must apply during the application periods for new undergraduates;
check for deadlines
on the Admissions website and review the selection
requirements.
Notify the Fee Waiver Coordinator (lhensley@calpoly.edu)
that you have applied for admission to Cal Poly. The coordinator
will provide the Admissions Office with an authorization form that
confirms your fee waiver eligibility and requests that the $55
application fee be waived.
The Admissions Office
will notify you when your application has been processed and you
have been admitted to the university.
STEP 4: Complete the Individual Career Development Plan (HR 133a form), obtain the required approvals, and submit the form to Human Resources as soon as you have been admitted to the University.
Pay the current quarter
Registration Fees at the State Cashier's Office (Administration
Bldg., Room 131-E) at least three (3) working days before you are
scheduled to register. Go to: Employee
Fee Waiver Tuition Amounts.
Register for classes
online (POWER) in accordance with the “Priority and Graduate
Students” registration category.
After registering for
classes, complete the Employee
Fee Waiver Course Notification (HR 133 form), obtain the
required approvals, and submit the form to Human Resources by the
current quarter.
If your class schedule
changes after you have submitted the HR 133 form, complete a Change
of Course Form (HR 133b form), obtain the required
approvals, and submit the form to Human Resources by the current
quarter.
