All new Instructional Student Assistants must bring a completed ISA-101 to the Payroll Services office and complete a new hire intake before they can begin working. ISA-101's are completed by the hiring department and submitted to the Academic Personnel office. If you have any questions about the hire process, please review the instructions on the ISA-101 form or contact your department.
In addition to a completed ISA-101, new Instructional Student Assistants should bring documents to complete an I-9 and their original Social Security Card to Payroll Services for their new hire intake. The I-9 form verifies your abilitiy to legally work in the United States and is required by Federal Law. Acceptable forms of identification are listed on the fourth page of the I-9 form posted below. Above and beyond satisfying the I-9, all state employees, including Instructional Student Assistants, are required to provide a valid social security card before they can receive pay.
Payroll Services is located in the Administration Building, Room 107.
All forms are available below. You will need Acrobat Reader to print out the PDF forms. It is available free here.
