Withholding Change Form

The number of allowances you claim determines the amount of Federal and State tax withheld from your paycheck. If you have questions regarding what you should claim, please contact your tax consultant or go here for more information.

If you are not a U.S. Citizen, you may not use this form. Any changes to your payroll withholding status need to be completed in the Payroll Services Office, building 1, room 107.

Please Note: If you are eligible to claim EXEMPT from withholding, claim more than 9 allowances, or wish to have specific additional amounts withheld you must make this change in person at the Payroll Office.
